REQUEST FOR PROJECTS

You are eligible to submit a funding request to the Fondation Santé Baie-des-Chaleurs if:

your request concerns the Baie-des-Chaleurs local service network
your request improves the quality and/or accessibility of health care
your equipment request is for a group of people or can be used by several users
your request is not a device whose replacement is planned by the ministry or will have to be;
your request is impossible to finance with the budgets granted to you by the CISSS de la Gaspésie or it is only possible to finance it in part using your budgets.

THIS IS THE PROCESS

September to December

1. Complete an application form for a medical project or device.

2. Have the people identified on it sign the form.

3. Submit the form to the Foundation, by email or in person at the Foundation office, with the attached submissions.

January

4. The general director of the Foundation assesses the relevance of the requests in relation to the criteria of the CISSS de la Gaspésie (validation with senior management)

FEBRUARY

5. The Foundation's project committee selects the projects and medical devices that can be financed according to the budget available for the current year.
